Salary Range:

Qualifications:

MINIMUM REQUIREMENTS:

A Bachelor's degree in Social Work with certification through the Wyoming Mental Health Professions Licensing Board. Experience with mental illness and substance abuse preferred.

Duties and Responsibilities:

1. Provides initial and ongoing assessment to determine case management needs of the client. Completes ASI/ASAM as assigned on individual cases.
2. Provides case management designed to provide goal-oriented and individualized supports focusing on improved self-sufficiency and recovery for the persons served through life skill functional assessment, planning, linkage, advocacy, coordination, transportation, housing, financial, health care and monitoring activities, supportive counseling and crisis intervention.
3. Maintains primary therapeutic responsibility for assigned clients who currently need case management and medication management but not clinical treatment services. May refer case management clients for treatment services as needed.
4. Receives referrals from primary therapists on the clinical and case management for linkage with entitlement programs, housing, medical, social, educational, vocational, guardianship, payee, substance abuse services, transportation, outside treatment and skill training resources in the community.
5. Monitor progress and effectiveness for clients in achieving treatment goals, stabilization of symptoms, and response to changes in service or treatment.
6. Will perform crisis stabilization for individual clients and assist with team clients as requested.
7. Serve as a client advocate, both individually and systemically, to ensure client rights are protected and to reduce stigma.
8. Be familiar with community resources, develop and maintain positive relationships with them.
9. Maintain timely documentation as required.
10. Participate in team and resource meetings, Attend supervision weekly.
11. Know and comply with agency policy and procedures, including code of ethics.
12. Other duties as assigned by supervisor, ARS Program Manager, Clinic or Executive Director.
