Substance Abuse Case Manager - Chrysalis House
Peak Wellness Center

Salary Range:

$23,600 + - health, dental, vision, life, retirement, STD, LTD, Aflac, FSA, paid vacation, holiday and sick leave

Projected Start Date:

July 15, 2007

Location:

Pine Bluffs, Wyoming

Qualifications:

MINIMUM REQUIREMENTS:

Bachelors' Degree in psychology, social work, psychiatric nursing, or related field.

Duties and Responsibilities:

1. 1. Develop and maintain positive working relationship with agency staff and other community providers.
2. Coordinate with other Peak Wellness Center staff and other community providers in completing the clinical assessment and treatment plan.
3. Prepare needed reports as requested in a timely manner
4. Complete intakes, set up file and assist in ASI's as requested for all residents of Transitions Complex.
5. Staff and refer clients needing treatment to outpatient, residential and other services in the agency and/or community providers.
6. Maintain case records as requested by the Clinic. Document all case management contacts/services in case files.
7. Provide emergency services during working hours and after hours as assigned.
8. Attend staff meetings, and other activities, which ensure the smooth functioning of clinic operations.
9. Attend weekly staff meetings to review cases, and as requested by TC Director.
10. Continue to acquire additional training and knowledge within the substance abuse field.
11. Other duties as assigned by supervisor, Director of Adult Services, Clinic Director/Executive Director.

Other responsibilities:
1. Close communication with clinicians and other case manager(s) to coordinate ancillary service needs for the clients.
2. Create and locate needed ancillary services for individual clients to meet their treatment needs. (Includes visits to other referral sources and services delivery sources to learn resources).
3. Assist clinicians with group management, as requested.
4. Provide outreach to potential clients, referrals, etc. (Judges, DFS, Probation & Parole, OYA, Pathfinder, Schools, DVR, FCS, etc.)
5. Assist clients in completing application for services process, as requested.
6. Accompany client to appointments, as needed.
7. Assist client and family in commitment for treatment.
8. Provide and arrange transportation, childcare, etc., to assist client to facilitate appointments for treatment services.
