Qualifications:

MINIMUM REQUIREMENTS:

A Master's Degree in social work. Wyoming Professional License or willingness to pursue. Training and/or experience in Geriatric Social Work is beneficial.

Duties and Responsibilities:

1. Receives referrals for services. Refers and facilitates inappropriate referrals to appropriate community service provider(s).
2. Provides out-reach and community based services to individuals who are identified as at risk.
3. Provides initial and on-going assessment to determine needs of the client. Completes additional screening and evaluation as is needed (i.e. mini mental status exam, geriatric depression screening, substance abuse evaluation, suicide risk).
4. Provides case management, educational and clinical services for clients individually and/or in a group setting.
5. Provides program and case consultation and education services internally and participates in agency program development activities. Works as part of a clinical treatment team.
6. Provides linkage for clients and/or service providers to secure access to services.
7. Monitoring/follow up: contacting the client to ensure a Treatment Plan is being followed and to monitor the progress and impact of that plan.
8. Advocacy on behalf of the specific client for the purpose of accessing needed services.
9. Maintain primary therapeutic responsibility for assigned clients who are receiving treatment, case management and medication management.
10. Maintain case records to reflect the course of treatment and provide the required documentation in adherence to PWC policies and procedures.
11. Attends staff meetings, supervisory conferences and other activities which ensure the smooth functioning of clinic operations and the clients we serve.
12. Knowledge and compliance with agency policies and procedures including the code of ethics.
13. Develop and maintain positive working relationships with other community providers. Coordination with PWC staff and other providers to ensure access and placement for clients.
14. Education and training of family members and other community providers to assist in early identification and intervention on behalf of older adults who are at risk.
15. Assist care givers in obtaining respite, support and access of services for those in their care.
16. Other duties as assigned by supervisor, Director of Adult Services, Clinic/Executive Director.
