Qualifications:
MINIMUM
REQUIREMENTS:
Bachelors
degree social work, with a part of training a supervised experience
providing mental health services. Two years of post degree
supervised experience in providing mental health services
may be substituted for practicum/internship. Wyoming Professional
License or willingness to pursue. Must have valid driver's
license and insurance.
Duties
and Responsibilities:
1.
Provides initial on-going assessment of client needs for treatment,
support, and skill training to maximize the client's abilities
to live as independently as possible in the community.
2. Provides comprehensive treatment planning with participation
of the client and based upon client goals.
3. Provides on-going therapy, rehabilitation, and skill training
to improve emotional, behavioral, daily living, self-care,
and cognitive skills.
4. Coordinates individual treatment with medication management,
residential, vocational, and day treatment services.
5. Develops and coordinates resources in the community that
support the client in activities of daily living, necessary
financial supports, and other services that may be necessary
for the client to maintain independence in the community and
reach his/her overall rehabilitation goals.
6. Provides flexible services geared to the client's needs.
7. Provides services for the client in his/her home and community.
8. Provides support and education to family members.
9. Serves as an advocate for the client and educates persons
in the community about serious mental illness as needed to
decrease stigma and increase opportunities for client to participate
within the community.
10. Provides crisis services as needed to meet the client's
needs and prevent unnecessary trauma and hospitalization.
Additional Duties and Responsibilities
1. Receives referrals for services. Refers inappropriate referrals
to appropriate community service provider(s).
2. Conducts intake assessments. Completes required documentation
and develops initial Treatment Plans.
3. Provides individual, marital, family, group and rehabilitative
services as appropriate to client needs. Make appropriate
referrals as needed during course of treatment.
4. Develops and maintains positive working relationship with
other community providers. Coordinates with other Southeast
Wyoming Mental Health Center staff and other community providers
in providing joint treatment as reflected in treatment plan.
Prepares needed reports of treatment as requested.
5. Maintains case records to reflect course of treatment and
provide required documentation. Provides follow-up and aftercare
planning services as needed. Expeditiously discharges inactive
clients.
6. Provides emergency services during work hours and after
hours as assigned.
7. Provides program and case consultation and education services
internally, and to other community groups and individuals.
Participates in agency program development activities.
8. Provides liaison service for local, regional and state
hospitals to ensure continuity of care for Center clients.
9. Maintains therapeutic responsibility for the collection
of fees.
10. Attends staff meetings, supervisory conferences, and other
activities, which ensure the smooth functioning of clinic
operations.
11. Provides mental health/substance abuse evaluations and
prepares reports of findings and recommendations.
12. Other duties as assigned by Clinic/Executive Director.
Other Duties and Responsibilities
1. Outreach - community rounds to identify and engage those
appropriate for services, as well as regular contact with
established clientele whom are resistant to services. Contact,
build rapport, and provide crisis services.
2. Psychological screening and diagnosis.
3. Facilitates referrals, as appropriate to hospitals and
primary health care.
4. Also includes habilitation and rehabilitation therapy,
Case Management including therapy, skill training and system
intervention with contact on a regular basis.
5. Provides substance abuse services to include psycho-educational
groups and individual therapy.
6. Referring and supporting access to income services.
7. Referring and supporting access to housing services.
8. Develops and maintains a positive working relationship
with the community providers.
9. Maintains case records to reflect course of treatment and
provide documentation.
